The project is aimed at providing support in a home environment for people with disabilities and elderly people dependent on care. The activities which are foreseen by the project will cover a minimum of 95 people who are representatives of the target group - adults unable to care for themselves and people with disabilities.
The present project will build on the implemented activities and the results achieved under the successfully implemented projects of the municipality of Dryanovo under the procedures "Patronage care for the elderly and persons with disabilities" and "Patronage care +" and for the representatives of the target groups support will be provided through the implementation of the following activities :
1. Provision of hourly mobile integrated health and social services at homes and psychological support for elderly people unable to care for themselves or people with disabilities;
2. Delivery of food, foodstuffs and basic necessities, transportation for the assistants to and from the individuals' homes, payment of household bills or requesting and receiving urgent administrative and household services (with user funds);
3. Providing the individuals from the target group with information about prevention and prophylaxis of health and socially significant diseases, including consultations with a nutritionist and other types of specialists;
4. Providing supervision and training to employees providing hourly mobile integrated health and social services in a home environment.
The implementation of the activities will be realized by forming a Team of specialists, including domestic helpers, medical personnel, a psychologist and others depending on the individual needs of the persons. Appointees will be supported through training (introductory and refresher) and supervision (individual and group). The service will be assisted by providing transport to the staff providing the services to and from the individuals' homes.

Предоставяне на мобилна интегрирана здравно-социална услуга "грижа в дома": Дейността е насочена към предоставяне на мобилни интегрирани здравно-социални услуги в домашна среда за срок от 18 месеца. Предвижда се да бъдат обхванати минимум 95 лица, представители на целевата група - възрастни в невъзможност за самообслужване и хора с увреждания. За представителите на целевите групи ще бъде осигурена подкрепа чрез реализиране на следните дейности: 1. Предоставяне на почасови мобилни интегрирани здравно-социални услуги по домовете и психологическа подкрепа за възрастни хора в невъзможност за самообслужване и лица с увреждания ; 2. Доставка на храна, хранителни продукти и продукти от първа необходимост, както и осигуряване на транспорт на персонала, предоставящ услугите, от/до домовете на лицата. Заплащане на битови сметки, заявяване и получаване на неотложни административни и битови услуги (със средства на потребителите; 3. Предоставяне на лицата от целевата група на информация относно превенция и профилактика на здравето и социално значими заболявания, включително консултации с диетолог и др. специалисти при необходимост; 4. Осигуряване на супервизия и обучения на служителите, предоставящи почасови мобилни интегрирани здравно-социални услуги в домашна среда. Ще бъдат осигурени въвеждащи и надграждащи обучения за назначените лица, които не са минавали подобно или същото обучение. Предвижда се провеждане на групови и индивидуални супервизии през целия период на реализиране на дейностите.
