The project proposal is aimed at providing support in a home environment for persons with disabilities and elderly people dependent on care in the territory of the municipality. The possibilities for providing social and integrated health and social services in a home environment will be expanded. The activities envisaged by the project will cover a minimum of 60 persons, representatives of the target group - adults unable to care for themselves and people with disabilities.
The project will supplement and upgrade the implemented activities and the results achieved under the implemented projects of the municipality under the procedures "Patronage care for the elderly and persons with disabilities" and "Patronage care +" and support will be provided for the representatives of the target groups through the implementation of the following activities:
1. Provision of hourly mobile integrated health and social services at homes and psychological support;
2. Delivery of food, food products and basic necessities, payment of household bills, requesting and receiving urgent administrative and household services with consumer funds;
The implementation of the activities will be realized through a formed team of specialists such as domestic helpers, a medical person, a psychologist and others depending on the individual needs of the persons identified on the basis of a needs assessment. The appointed persons will be supported by providing the necessary training (introductory/upgrading) for the performance of their activities, as well as by conducting supervisions.

Предоставяне на почасови мобилни интегрирани здравно-социални услуги.: Изпълнението на Проект "Грижа в дома в Община Оряхово" ще осигури продължаване и надграждане на създадения модел за предоставяне на почасови мобилни интегрирани здравно-социални услуги за възрастните хора и лицата с увреждания.Предвидените по проекта дейности ще обхванат минимум 60 лица, представители на целевата група - възрастни в невъзможност за самообслужване и хора с увреждания.
Дейността включва:
- Подбор и назначаване на персонал за предоставяне на услугите.
- Обучение на персонала за предоставяне на услугите.
- Изработване на индивидуални планове за социални и здравни услуги за потребителите .
- Ежедневно предоставяне на услуги от наетите за целта лица.
- Оценка и актуализиране на индивидуалните планове за социални и здравни услуги.
- Поддържане на база данни и досиета.
- Мониторинг на услугите в домовете на потребителите.
- Оценка на работата на персонала.
Дейността е с продължителност 19 месеца,един от които е за подготвителни дейности и 18 за предоставяне на услугите.
