                                    Activity: Предоставяне на почасови мобилни интегрирани здравно- социални услуги за нуждаещи се лица с увреждания и възрастни хора в невъзможност за самообслужване, супервизия и указания към екипите във връзка с въвеждането на новия инструментариум за оценка на потребностите и работа с целевата група: Дейността включва предоставяне на почасови мобилни интегрирани здравно- социални услуги за нуждаещи се лица с увреждания и възрастни хора в невъзможност за самообслужване. Услуги ще получават 24 лица.

Предоставянето на услугите ще започне от 01.06.2025г., като от 01.05.2025 г. до 31.05.2025 г. ще бъде подготвителен период за извършване на оценките на потребностите на потребителите.,
