                                    Activity: Дейност 1 - Предоставяне на почасови мобилни интегрирани здравно- социални услуги, супервизия и указания към екипите: Основната цел на дейността е продължаване на създадения модел за грижи в домашна среда на територията на община Кубрат, чрез предоставяне на почасови мобилни интегрирани здравно- социални услуги за нуждаещи се лица с увреждания и възрастни хора в невъзможност за самообслужване. Услуги ще получават най малко 60  лица.
Етапи за изпълнение на дейността:
1. Подбор на целевата група:
- самотноживеещи лица с увреждания, които независимо, че нямат определена чужда помощ, разполагат със съответните медицински документи, удостоверяващи невъзможността им за самообслужване за определен период от време; 
- самотноживеещи лица в надтрудоспособна възраст в невъзможност за самообслужване, които нямат определена по съответния ред степен на намалена работоспособност.
2. Подбор на персонал за предоставяне услуги по грижа в дома;
3. Предоставяне на почасови мобилни интегрирани здравно - социални услуги за нуждаещи се лица с увреждания и възрастни хора в невъзможност за самообслужване, супервизия и указания към екипите във връзка с въвеждането на новия инструментариум за оценка на потребностите и работа с целевата група.	,
