                                    Activity: Предоставяне на почасови мобилни интегрирани здравно- социални услуги за нуждаещи се лица с увреждания и възрастни хора в невъзможност за самообслужване, супервизия и указания към екипите във връзка с въвеждането на новия инструментариум за оценка на потребностите и работа с целевата група: Община Кочериново им значителен опит при предоставяне на интегрирани социално-здравни услуги в домашна среда. Реализирани са проекти по процедурите за патронажа грижа и грижа в дома. Потребителите са удовлетворени и услугите са търсени от лицата.
Здравно-социалните услуги доказаха своята ефективност и ефикасност в община Кочериново. Предоставяне на почасови мобилни интегрирани здравно- социални услуги за нуждаещи се лица с увреждания и възрастни хора в невъзможност за самообслужване. Услуги ще получават 22 лица 
Дейността по предоставяне на интегрирани здравно-социални услуги стартира от 02.01.2025г.,
