Description

Brief description 
Целта на проектa е да подобри качеството на живот и възможностите за социално включване на хората с увреждания и възрастните хора в общината, чрез осигуряване на мрежа от услуги в домашна среда и изграждане на подходящ материален и кадрови капацитет за предоставянето им чрез въвеждане на модел за патронажни грижи за възрастни хора и лица с увреждания, вкл. с хронични заболявания и трайни увреждания за осигуряване на почасови мобилни интегрирани здравно-социални услуги в техните домове.
Activities
  • Activity: Предоставяне на почасови мобилни интегрирани здравно-социални услуги: Дейността ще усъвършенства и подобри достъпа и услугите в общността за хора, които поради различни ограничения от здравословен характер са изключени от социалния живот и са в риск от включване в институционален тип грижи. Патронажната грижа ще допринесе за разширяване на социалните услуги, предоставяни в общността и за подобряване качеството на живот на хората от уязвимите групи и/или техните семейства. Чрез нея ще се осигури мрежа от услуги в домашна среда, която ще подобри възможностите за социално включване на хората с увреждания и възрастните хора. Дейността ще изгради модел за патронажни грижи за възрастни хора и лица с увреждания, вкл. с хронични заболявания и трайни увреждания с цел осигуряване на почасови мобилни интегрирани здравно-социални услуги в техните домове., Contracted Amount: 0.00 , Reported Amount: 275 986.59
